Case Manager
Provide case management to adults and children focusing on strengths of individuals and families. Case management services assist individuals in gaining access to needed medical, social, educational, and supportive services. Case Management services include assessment, linkage/referral and follow-up based on individual need. Case management is primarily a home and community-based service designed to be conducted within natural settings.
Employee is responsible for completing all documentation regarding client services in accordance with VBHCS P&P QM-GEN-001 - Signing of Clinical Documentation; attending staff meetings and supervision sessions as required; completing required training as assigned and/or scheduled; meeting the productivity billable unit standards as assigned by VBHCS; and monitoring their billable services report.
Estimated Salary: $20 to $28 per hour based on qualifications.
